Vital Records

These documents according to the Illinois State Law (410 ILCS 535 – Vital Records Act) include birth, death, and marriage records. All of these documents are kept in the office of the County Clerk and the state of origin. Vital records maintained in this office can be searched from the early 1800s to the current year. This office will certify copies of these documents to eligible individuals applying for them.
How to Obtain Vital Records
To obtain certified copies of these forms you must apply for them in the following ways:

Forms
Below are downloadable applications and those who are authorized to receive said documents:
- Birth Application (PDF) - The person themselves, if over 18 years of age, or parent or legal guardian of the child
- Birth of Deceased Application (PDF) - Parent, legal guardian or surviving spouse (must have certified death certificate)
- Civil Union Application (PDF) - Open to the public
- Death Application (PDF) - A person with a financial interest or any relative
- Marriage Application (PDF) - Open to the public
- Vitals Application (PDF) - Determined by types of records requested
Fees
Effective July 1, 2012, an additional fee of $2 will be collected by all local registrars and county clerks for the issuance of certified copies of death and fetal death certificates.
PA 97-0679 requires this additional fee to be put into the Cemetery Oversight Licensing and Disciplinary Fund (COLDF) and used by the Illinois Department of Financial and Professional Regulation (IDFPR).
